Form 990: Resources for Tax-Exempt Organizations

Tax-exempt status allows civic, charitable, and religious organizations to devote their resources to their missions, but maintaining that status can be challenging. Many organizations do not have the expertise or time internally to manage reporting and tax requirements. In particular, the Redesigned Form 990 is more complex than ever before. Board Questionnaire Template

The Redesigned Form 990 seeks more disclosures from tax-exempt organizations than ever before. Multiple areas of the Form require information on trustees/directors, officers and key employees (a group commonly referred to as "managers"). It also seeks information on those individuals’ family members and business holdings. The Form 990 Instructions specify that an annual questionnaire may be used as evidence of an organization's "reasonable effort" to collect this information.

Clark Nuber and Eve Borenstein have created a Board Questionnaire to help you meet the conditions the IRS has provided in the "reasonable efforts" examples in the Form 990 Instructions. This interactive Microsoft Office Excel workbook helps you determine who should receive the questionnaire and creates an individual questionnaire for each recipient which can be printed or distributed electronically. It also includes the components of a “reasonable effort” questionnaire for other individuals specified in the Form 990 Instructions such as grant selection committee members and substantial contributors.

Form 990 Organizer

Our interactive tax organizer makes it easier for you to collect only the information you need to file Form 990. Arranged in the same sequence that a tax return should be prepared, it starts with a series of questions that determine which information is required. Additional features include:
